Memory Cafe – Havant & Waterlooville Primary care Network

Havant and Waterlooville Primary Care, Dementia Support Team staff are running a Memory café in Denmead at the All Saints Church, Hambledon Road, on the 2nd Wednesday of every month, 11:00-12:30pm.

They also run a memory café in Havant at the Pallant Centre, PO9 1BE, on the 3rd Thursday of every month 11-12:30.

There are refreshments, some times entertainment/speakers, useful information, and a chance to chat to others in similar positions, as well as getting help and advice from our dementia coaches.

Disabled Facilities Grants – Fareham Borough Council

If you are having difficulty living at home because of a disability, we may be able to help you to carry out essential adaptations so that you can continue to live independently. This work can be funded by a disabled facilities grant. You can apply for it if you own your own home, rent or live with your family.

Housing Benefit provides help with paying your rent.  The amount of help to which you may be entitled will depend on your income, savings and family circumstances.

You can apply for Housing Benefit if you pay your rent to a private landlord, to a Housing Association or if you are a council tenant. Your income and circumstances are taken into account. Personal Independence Payment (PIP)

You may be able to get help with some of the extra costs caused by long term ill-health or disability.

The amount you get depends on how your condition affects you, not the condition itself.

You’ll be assessed by a health professional to work out the level of help you can get. Your rate will be regularly reviewed to make sure you’re getting the right support.

Carer’s Allowance

You could get a weekly carer’s allowance if you care for someone at least 35 hours a week and they get certain benefits.

You don’t have to be related to, or live with, the person you care for.  You won’t be paid extra if you care for more than one person.

Attendance Allowance

You may be able to claim Attendance Allowance if your ability to keep safe or look after your own personal care is affected by physical or mental illness or disability. It doesn’t cover mobility needs.

Attendance Allowance has 2 weekly rates, and the rate you get depends on the help you need.

Attendance Allowance won’t reduce any other income you receive, and it’s tax-free. You don’t have to have someone caring for you in order to claim, and you can use the money to help with extra costs you have due to disability. You can get an Attendance Allowance claim form by calling the Attendance Allowance Service Centre or claim online.

 

 

Pension Credit

Pension credit is available to people who have a low pension income. Additional amounts could be available for severe disability or carer responsibilities.

Pension Credit can be applied for over the telephone. Quite a lot of information is needed so have your national Insurance number, details of any income, savings, rent and bank account details ready before making the call.

Friends of Fareham Community Hospital

The Friends of Fareham Community Hospital have been formed to give support to the new Hospital and help to link it with the community which it serves. We help to provide guidance to users on entering the hospital, operate a dementia friendly memory club, operate the blood test desk, raise funds for equipment and materials which are outwith the limited budget of the NHS and raise awareness of the services available at the hospital.

FFCH as we call it was started in June 2010 soon after the Hospital was established. Set up as a charity in September 2011, the Friends’ initial objectives are:

Concessionary Travel

The older person’s bus pass entitles you to free travel on bus services throughout England, including London, from 9.30am to 11pm Monday to Friday and all day at weekends and on public holidays.

The disabled person’s bus pass entitles you to free all day travel on bus services on journeys starting in Hampshire (not Portsmouth and Southampton) regardless of where you are travelling to. Free travel between 9.30am to 11pm Monday to Friday and all day at weekends and on public holidays on bus services throughout the rest of England (including London).

The disabled person’s bus pass plus companion entitles you to free all day travel on bus services for pass holder and companion on journeys starting in Hampshire (not Portsmouth and Southampton) regardless of where you are travelling to.  Free travel, for the pass holder only, 9.30am to 11pm Monday to Friday and all day at weekends and on public holidays on bus services throughout the rest of England (including London). Companions must pay for their journey.

Travel vouchers are an enhancement to the scheme for disabled people who are unable to use a bus service. Travel vouchers are an alternative to the bus pass. You cannot have both. To use community transport services we recommend you apply for a disabled person’s bus pass.  Travel vouchers do not entitle you to discounted fares, you will need to pay the full fare for all services.

The Real Junk Food Project (South Coast)

We are an environmental organisation who are tackling food waste. We collect ‘food waste’ that is still fit for human consumption from various places and offer it to ANYONE & EVERYONE in our cafes or shopping boutiques. A shopping boutique is a place you can shop; it is surplus food that we are unable to cook with that day/week. We provide a pay as you feel system, you decide the value of your food and our service. At our branch you can do that by offering time, cash, skills or item donations.  Pop up Cafes: Everyone is welcome to the cafes; it is open to the public. You don’t have to fit into a category to eat with us.

Fareham & Gosport Basics Bank

This service is accessed through over 25 agencies in the area that come into contact with people that are likely to require assistance, such as Health Visitors, Probation Service, Social Services, Job Centre Plus, Council Housing offices, Fareham and Gosport Family Aid, homeless charities, etc. People may access the service through one of the many referral agencies in the Fareham and Gosport area up to four times in one year.  This is to prevent them from becoming dependant on the service and also ensures that we can support as many people as possible.  We rely on the agency to decide the need and to manage the relationship with their client.

Direct Payments

If you are eligible for care through Adult Services, and do not wish to use the care provided, Direct Payments is a payment from Hampshire County Council used to pay for care or services arranged by you, or someone acting on your behalf. You should contact your social worker to discuss further.

Blue Cross Pet Bereavement Support Service

The mission of Blue Cross Pet Bereavement Support Service (PBSS) is to provide free and confidential emotional support and information to anyone who is affected by the loss, or anticipated loss, of a pet or assistance animal through illness, euthanasia, natural death or other circumstances like theft or attack.

We commit to providing this support service every day of the year from 8.30am to 8.30pm.

Veterans Welfare Service

VWS is run by the Ministry of Defence’s (MODVeterans UK and provides free one-to-one support to veterans or anyone supporting a veteran, their families and dependants, with a national network of welfare managers across the UK and Republic of Ireland.

Veterans Outreach Service

Veterans Outreach Support (VOS) provides a monthly Drop-In meeting and weekly mental health therapies for former members of the British Armed Forces and Reserves and their direct family members, including the Royal Fleet Auxiliary and the Merchant Navy.  It is a welcoming and relaxed place providing social support and access to a range of welfare and mental health services.

RBL provides lifelong support for the armed forces community including those serving, veterans and their families.

We support serving and ex-serving personnel all year round, every day of the week.

Care after Combat
The Care after Combat Team consists of a small number of full-time and part-time staff, working together with volunteers across an extensive area of operations. We are all committed to fulfilling our mission; to provide support within the Criminal Justice System for the well-being of former and serving British Armed Forces personnel and their families.
Alabaré Homes for Veterans

Our Hampshire Homes for Veterans offer housing and support to ex-Service personnel who are homeless or are at risk of becoming homeless.  All our residents must have served in the British Armed Forces.

In Hampshire we currently have homes in Gosport, with 16 bed spaces in supported accommodation, providing a low to medium support, and 3 bed spaces in move-on accommodation where you can live more independently while learning the skills you need to maintain your own home.  Our largest Home for Veterans in Gosport, Greenwich House, has been made possible thanks to the generous support of Greenwich Hospital.
