Making health connections is about people getting the support they need to enable them to self-manage and make improvements to health and wellbeing. This website provides a place for people to easily find out about local services and groups to support health and wellbeing, and has been developed by Health Connectors working for Southern Health NHS Foundation Trust.
Health Connectors and Care Planners work across primary care, community healthcare teams and other services to support individual self-management. We do this by working with people to understand their needs, and develop a plan to support and enable people to achieve their personal goals. Navigating services can be difficult, especially during times when people may feel unwell or stressed. We support people by working across local health teams and other services to ensure that care and support is co-ordinated where that is needed.
The service is being developed as part of an Extended Primary Care Team and will initially focus on working with people that have complex health and wellbeing needs. If you would like more information please contact us.
